A large number of doctors in the health service are not in training posts but do not work as consultants or principals in General Practice. This group, known as SAS Doctors, is diverse and heterogeneous, includes virtually every specialty and encompasses doctors with a variety of backgrounds, levels of training and responsibilities. They are almost universally involved in delivering patient care with many Trusts relying heavily on their input to develop and maintain services.

Most were employed as ‘Staff grade’ or ‘Associate Specialist’ (SAS) doctors in the past and some continue in their old contracts, hence the term SAS doctors is preferred. A new grade – ‘Specialty doctor’ was established with the introduction of the ‘new contract’ in 2008. Many SAS doctors chose to transfer to this new contract and all new appointments to the grade are now at ‘Specialty doctor’ level.
